These Terms refer to the following additional terms, which also apply to your use of our site:

The site and the Services

This site provides an evidence-based risk assessment tool for screening of older people in health & social care settings (eg, residential care homes, intermediate care units). It is intended to be used by professional care staff, with the support of appropriate training and guidance. You must be employed by a recognised organisation to access this app and use it for its intended purpose.

The site is intended to provide an individual care plan for residents/patients, based on the answers provided by the member of staff responsible for carrying out the risk assessment. Other than information received directly from a health & social care worker, the site’s content is not medical advice. You should always talk to an appropriately qualified health care professional for diagnosis and treatment. You must obtain professional or specialist advice before taking, or refraining from, any action on the basis of the content on our site.

Although we make reasonable efforts to update the information on our site, we make no representations, warranties or guarantees, whether express or implied, that the content on our site is accurate, complete or up to date.

The online risk assessment tool includes the following services (collectively the Services):

  1. access to a site that acts as a simple checker to allow care home managers and staff to keep track of  Covid symptoms for their residents on a daily basis, whilst allowing clinicians to log onto a dashboard to access real-time data, straight from the care homes.

  2. the ability to report and capture relevant information about Covid test results, vaccination information and Advance care planning.

  3. a set of care plans and reports to enable ongoing monitoring and reporting of assessments.

THE SITE IS NOT MONITORED AND YOU SHOULD BE AWARE THAT ELECTRONIC COMMUNICATION WITH CARERS WILL NOT BE REVIEWED IN REAL-TIME.

Your use of the site and the Services

To access the site and use the Services available via the site, you must be employed by a CQC registered care organisation in England, Scotland, Wales or N.Ireland. We do not represent that content available on or through our site is appropriate for use or available in other locations.

You agree to use this site only for lawful purposes, and in a manner which does not infringe the rights, harass, threaten, or restrict, or inhibit the use and enjoyment of the site by any third party.

In addition, without limitation, you agree that you will not do any of the following while using or accessing the site and/or the Services:

  1. upload, post, email or otherwise transmit any content that infringes the intellectual property rights or violates the privacy rights of any third party (including without limitation copyright, trademark, patent, trade secret, or other intellectual property right, or moral right or right of publicity);

  4. circumvent, disable, or otherwise interfere with security-related features of the Services or features that prevent or restrict use or copying of any content; and/or

  5. attempt to probe, scan or test the vulnerability of any site system or network or breach or impair or circumvent any security or authentication measures protecting the Services.

We reserve the right to suspend or terminate your access to the site and the Service if, in our opinion, you are in breach of the above requirement.

Use of the Services by or on behalf of residents/patients
